Welcome to our Partial Hospitalization Program (PHP) at Sage Recovery and Wellness Rehab in Austin, Texas, where we are dedicated to guiding individuals towards healing, recovery, and lasting well-being.
Partial Hospitalization Program (PHP) is a powerful and effective solution for those seeking intensive treatment for mental health issues or substance use disorders. At Sage, we understand that every journey to recovery is unique, and our PHP services are tailored to address your specific needs. Our PHP is designed to offer a balanced and structured approach to mental health and substance use treatment, providing intensive care while allowing individuals to reside at home.
At the heart of our PHP is a commitment to structured intensity. We provide a higher level of support than traditional outpatient care, ensuring individuals receive the comprehensive care needed for their unique challenges.
Our PHP offers a broad spectrum of therapeutic services, including individual therapy, group therapy, psychoeducation, and psychiatric services. This holistic approach addresses the multifaceted aspects of mental health and substance use concerns.
We understand the importance of flexibility in the recovery journey. While our PHP is structured, individuals have the independence to return home in the evenings, allowing them to practice learned skills in the context of their daily lives.
For those transitioning from inpatient or residential care, our PHP serves as a vital step-down option. It provides continued support as individuals progress in their recovery within a less restrictive setting.
Clinical support within our PHP is individualized to address the specific needs and goals of each participant. We believe in tailoring treatment plans to ensure they align with the unique challenges individuals may be facing.
Acknowledging the interplay between mental health and substance use issues, our PHP is equipped to address dual diagnosis or co-occurring disorders. We provide specialized care that addresses both aspects simultaneously.
Family plays a crucial role in the recovery process. Our PHP emphasizes family involvement through therapy sessions, educational programs for family members, and support in creating a supportive home environment.
To ensure continuity of care, our PHP offers a more intensive level of support than traditional outpatient care. This helps individuals maintain progress, prevent relapse, and seamlessly transition to the next phase of their recovery journey.
Central to our PHP is the emphasis on skill-building. Participants learn and practice coping mechanisms, relapse prevention strategies, and life skills that are essential for long-term recovery.
Our PHP promotes community integration by allowing individuals to live at home and engage in treatment during the day. This approach helps bridge the gap between structured care and independent living.
A multidisciplinary treatment team, including therapists, counselors, psychiatrists, and other professionals, collaborates seamlessly to provide comprehensive and well-coordinated care.
Our PHP involves meticulous aftercare planning. This plan outlines the individual’s transition to less intensive levels of care, ongoing outpatient services, and community support to ensure sustained recovery.
When necessary, our PHP includes medication management as part of the treatment plan. This involves a thorough psychiatric evaluation, prescription management, and continuous monitoring of medication efficacy.
a) Stabilize and reduce acute signs and symptoms,
b) Increase functioning
c) Assist the client with integrating into community life.
Partial Hospital Programs are treatment programs held in a clinic or a hospital setting. A psychiatrist is in charge of the treatment.
A PHP is used when you are having mental health symptoms that require more help than outpatient therapy can provide, but do not require you to stay overnight in a hospital. PHP can be used to treat mental health conditions or can specialize in the treatment of co-occurring mental health and substance-related disorders. -Megan Simmons, LCSW
Criteria for admission to this level of care includes:
a) The client is not in imminent or current risk or harm to self, others, and/or property.
b) Assessment and diagnosis and/or treatment planning requires observation and interaction for at least 20 hours per week. This includes frequent interaction with the client and observation of the client with others. The treatment plan must be changed frequently which requires that the provider have face-to-face interactions with the client several times a week.
c) The client requires engagement and support which requires extended interaction between the client and the program. For example, the client requires a coordinated transition back into the community after treatment in Inpatient or a Residential Treatment Center, such as engagement with wraparound services or natural resources, due to the client’s inability to access or utilize family or other natural resources on their own.
d) The client requires a structured environment to practice and enhance skills. This requires face-to-face interactions several times a week that cannot be provided in a less intensive setting. These skills include maintaining their current living situation and/or returning to work or school.
e) The client requires a structured environment to complete goals and develop a plan for post-discharge services in a less intensive setting. Assistance may be needed to develop the skills necessary to self-manage medications and to make progress toward goals in spite of an environment that does not support recovery and/or limited community support services.
A Partial Hospitalization Program (PHP) is a structured, comprehensive, and intensive outpatient treatment program that provides individuals with the support and resources they need to overcome mental health challenges and addiction issues. PHP treatment is an excellent option for those who require daily therapeutic support but do not need the 24-hour care of an inpatient facility.
Recovery is possible, and it starts with taking the first step. If you or a loved one is struggling with mental health issues or substance use disorders, our PHP program is here to provide you with the tools and support you need to overcome obstacles and regain control of your life.
